Students must fill out this form in its entirety to request permission to register for Chem 50x0 or Chem 51x0.  The courses involve Independent research supervised by a faculty member with a weekly conference, a written and oral report. Research courses are taken in the sequence Chem 5010, Chem 5020 then Chem 5030 - OR - Chem 5110, Chem 5120 and then Chem 5130.

Once this form is received, the Chair or Deputy Chair of the Chemistry Department will review the information and confer with your proposed research advisor before emailing you with registration instructions.
