2023-2024 NORTH BELLMORE U.F.S.D.TECHNOLOGY DEVICE LENDING AGREEMENT 
The North Bellmore Union Free School District (“District”) hereby loans to the undersigned Student one iPad for the Student’s use during the school year in connection with school-related work and assignments subject to the following terms and conditions. Students in  grades two through six will travel to and from school each day with the device, or as required by his/her classroom teacher.


Student Use of Equipment:

The iPad will be distributed with a protective case or a keyboard case. The charger for the iPad will be housed in the classroom storage and charging cart, and therefore the chargers will remain in the classroom and not be sent home with devices.

All District-issued iPads are treated as school computers under the District’s Computer Network and Acceptable Use Policy and are to be used for school-related work according to your teachers’ instructions and the guidelines set forth below.  The District retains sole title and right of possession to the equipment.  The District retains the right to repossess and withdraw authorization for further use of the iPad without any prior notice. The District also retains the right to collect and/or inspect the iPad at any time without any prior notice and to alter, add or delete installed software.

North Bellmore implements a content filtering system that allows iPads to be strictly filtered regardless of whether the device is at home on a private Wi-Fi network, or in school on the district's network. Student iPads will retain strict filtering and content blocking regardless of the device location, or Wi-Fi network that it is joined to.



Student Responsibilities:

1. You must adhere to the District’s Computer Network Agreement and the Computer Network and Acceptable Use Policy provided to you by the District when using the iPad.
2. Inappropriate and/or illegal use of the iPad is improper and considered to be unauthorized and may result in suspension of your right to use the iPad and/or disciplinary action pursuant to the District’s Code of Conduct and/or referral to law enforcement as the situation may warrant.
3. You may not alter or delete school issued iPad applications (“apps”), school files, filters or system preferences on the iPad without your teacher’s express permission.  
4. You may not download social networking applications, including but not limited to Facebook, Instagram, Twitter and the like.
5. You must bring your iPads to school fully-charged every day of the school week or as required.
6. You are responsible for taking proper care of your iPad, both at school and at home. You must keep the iPad in a secure location when it is not at school. You must keep the equipment clean.
7. You are not permitted to remove any identification or serial numbers.
8. You are not permitted to let anyone use your iPad, other than your parents or guardians or teachers for reviewing school related work.
9. You are required to report any problems, damage or theft immediately to a teacher or staff member.
10. Any student owned applications, music, photos, and the like must be removed from the device prior to collection of the iPads by the District at the end of the year.  The District will not be responsible for loss of personal apps, music, photos, and the like on the iPad.

Parent Responsibilities:

1. You must review the content of your child’s iPad on a consistent basis to ensure that your child is utilizing the device in accordance with District policy and this iPad user agreement.
2. You individually and on behalf of your child waive and release the District, the Board of Education, its agents, officers and employees from any and all claims and damages of any nature arising from your child’s use of, or inability to use the school technology, including but not limited to claims that may arise from the unauthorized use of the iPad, including but limited to using the iPad for purposes unrelated to school work and assignments.
3. Although the district has implemented a strict content filtering protocol at the device level, you understand that it is impossible for the District to restrict access to all inappropriate materials which may be accessed from the iPad, and you will not hold the District responsible for materials accessed on the network.  You hereby individually and on behalf of your child waive and release the District, the Board of Education, its agents, officers and employees from any and all claims and damages of any nature arising from your child’s use of the iPad and material accessed on the network or Internet, including but not limited to content that may be considered inappropriate.  
4. You hereby individually and on behalf of your child also expressly waive the right to bring or pursue any complaint, lawsuit, administrative proceeding, appeal or review based upon any inappropriate content that may be accessed on the iPad, and/or any and all claims and damages of any nature arising from your child’s use of, or inability to use the school technology, including but not limited to claims that may arise from the unauthorized use of the iPad, including but not limited to using the iPad for purposes unrelated to school work and assignments.

District Responsibilities:  

The District reserves the right to:
1. Monitor iPad activity, including internet access or intranet access on the school's file servers.  
2. Make determinations on whether specific uses of iPads are consistent with the District's policies.
3. Suspend the student’s access to the District’s network and/or use of the iPad if at any time it is determined that the student is engaged in unauthorized activity or is violating District policies.
4. Violation of the District’s Computer Network and Acceptable Use Policy while using the iPad may result in disciplinary action pursuant to the District’s Code of Conduct and/or referral to law enforcement as the situation may warrant.
5. The iPad will be distributed with a protective case or a keyboard case. The charger for the iPad will be housed in the classroom storage and charging cart, and therefore the chargers will remain in the classroom and not be sent home.


Damage or Loss of Equipment and Optional Insurance

1. The approximate screen repair cost is $120.00. There will be a one-time accidental screen break replacement covered by the school district. Any screen broken after that initial repair paid for by the school district will be at the expense of the parent/guardian. 

2. In the event of any additional damage, theft, or loss, the parent/guardian will be fully responsible for the cost of repair or replacement.  All damaged equipment remains the property of the District. Any damage, theft or loss of or to the iPad must be reported to the District immediately.  Approximate replacement costs include $300 per iPad, and $100 per case 

3. It is the Student/Parent’s responsibility to return the iPad and all related equipment on the specified date  and in the same condition issued, with normal wear and tear excepted as determined by the District.

4. The full replacement cost of the equipment will be charged to the student’s family if the iPad and all related equipment are not returned within 30 business days of the due date.

5. Breach of the above rules may result in loss of the privilege of using the equipment
.
6. If the device is lost, stolen, abused beyond repair the family will be charged the current replacement cost of the device. Purchasing insurance coverage for a lost or stolen device is the choice and sole responsibility of the student’s family. Device insurance can be purchased through Worth Avenue, https://www.worthavegroup.com/product/ipad-insurance/. Coverage for one year is approximately $58.00 for $299.00 of coverage, which covers unlimited accidental damage, cracked screens, spills and theft. Please keep in mind, the district will cover the first screen repair. 


Repairs to Devices:

Should an iPad be broken or damaged while at home, repairs to district owned devices  must be repaired by the district's authorized repair facility, iProdigy. iProdigy is located at: 2281 Bellmore Ave, Bellmore, NY 11710. iProdigy offers common repairs including cracked screens or broken buttons at a reduced rate for all North Bellmore devices, as well as a lifetime warranty on the work they complete. Lifetime warranty on repairs is essential to keeping all North Bellmore devices working properly..
