By completing the following form, you are securing a confirmed* booking for your child(ren) on the following course:

Course Title: Multi Sports and Creative Activities Course (suitable for children aged 5 to 16 years)
Course Location: Gladstone Park Community Centre
Course Dates: Thursday 4th, Friday 5th April, Thursday 11th & Friday 12th April 2024
Course Times: 10am to 2pm each day (extended days available by prior arrangement and at no extra cost**)

**Earlier drop offs and later collections must be arranged and agreed prior to course attendance.

*Eligibility: Receipt of Free School Meals or PCC Eligibility check
  • Families receiving income-related free school meals (FSM)
  • Families where there FSM eligible and non-eligible children in the same household
  • Newly arrived refugee families
  • Families who are struggling financially (subject to referral from your child’s school)
  • Families supported by Social Care or Early Help Services (subject to referral from the service)
Full booking process: Opens on Monday 26th February 2024 at 9am
Course capacity: 40 (Highly likely to be 'sold out' within days)

Parent, Guardian or Carer Declaration:
I understand that the provider will share the information provided on this form where necessary with the county council within which the named child resides to check the child named on this form receives benefits-related Free School Meals (FSM)[1]. This check will be based on the information supplied by me and I declare that all details I have provided are true. I understand any false or incorrect information could result in the place being withdrawn, should it be awarded based on the information supplied.

By submitting this provision booking I give permission for the associated county council within which the named child resides to check my details on the Department for Education Eligibility Checking Service (ECS), for the purpose of checking my child’s eligibility for Free School Meals for access to the HAF programme.

[1] The HAF programme is for children eligible under Free School Meals (FSM) economic criteria only, not Universal Infant Free School Meals, which is a universal offer for children from Reception to Year 2).
