YOU MUST FILL OUT A NEW FORM EACH WEEK, AND THE NEW FORM WILL COME WEEKLY FROM SCHOOLMESSENGER.

PLEASE NOTE - DO NOT FILL OUT THIS FORM FOR VIRTUAL LEARNERS, THERE WILL BE A SEPARATE FORM FOR STUDENTS THAT ARE VIRTUALLY ENROLLED.

THE CUT OFF TIME FOR THIS BREAKFAST ORDER WILL BE WEDNESDAY at 2PM.  At that point the form will be turned off.

You MUST have your student's ID numbers to pick up your meals.  If you do not know those numbers, please contact Jessica Endres at jendres@tkschools.org.

This order form is for BREAKFAST for the week of May 31st.  These breakfasts will be debited from your child(ren)'s meal account based on their free, reduced, or full pay lunch status.  During the USDA free meals waiver, your child's account will be debited "$0.00" amount, and this extends to the remainder of the 20-21 school year.

PLEASE NOTE - This breakfast pick up will contain a complete nutritional breakfasts for the entire week, and MUST be picked up on THURSDAY.  Pick up will be at the Middle School, please come in through the service drive from the Green Lake Rd entrance FROM 3:30PM to 5:30PM.  Please use the bus loop upon entering the driveway.
