Made With Care - Maker Submission Form
Welcome!  We are so thrilled that you wish to contribute to Made With Care 2022.  Made with Care 2022 is an online fundraiser and awareness campaign that raises funds for The Mother Ship, Inc., a 501(c)(3) non-profit that helps parents of children with special needs in Central Ohio fulfill their own caretaker wellness and personal life goals.  In carrying out our mission, we noticed the remarkable maker skills of many caretakers in our community.  Some craft to relieve stress and have an outlet; others are making for a living, finding that applying their skills gives them the flexibility to care for their child's needs at the same time. 

Here's how you can contribute!  By filling out this form, you are agreeing that:
  1. You will donate an item you created for the Made with Care online charitable auction.  Items can be handmade, or otherwise generated from businesses owned or operated by caretakers of children with special needs.  (If you are not a special needs parent or caretaker and still want to contribute something, that's great too.  We will list you as a community supporter.)
  2. You will tell us (below) a little bit about yourself and what you make, and (with your permission) we will showcase it (and you!) on our online and social media outlets leading up to the event!  We are on a mission to tell the world about the works of caretakers in our community, which often remain hidden.  
  3. You will arrange to have your item in our hands no later than Sunday, November 13, 2022.  If you live in Franklin County, OH or one of its surrounding counties (this is our service area), we can arrange for pick-up.  If you live outside this area, we will contact you to discuss shipping.     
  4. PLEASE NOTE:  The sooner we receive your item, the sooner we can start showcasing you.

1.  The information you have provided on this form is true and accurate to the best of your knowledge.

2.  You will respond to our communications regarding picking up and showcasing you and your item.  You understand that we may not be able to showcase you or auction your item if we do not receive timely communication.  Items not in our hands by November 13 will not be included in the auction.  No exceptions.  

3.  As a Maker-Donor, we will provide information about you and the items and services you create online and in our social media outlets in order to provide awareness of the challenges faced by caretakers in our community, as well as to showcase your remarkable talents and encourage public support for what you do.  We cannot, however, advertise or endorse any private product or service due to legal restrictions on tax-exempt charities.  Thank you for understanding.  

4.  The items you are providing to Made with Care are donations.  You will not receive any remuneration in exchange from your donation, and agree that all proceeds from the Made With Care 2022 auction will be retained as charitable funds by The Mother Ship, Inc.  Your donation is tax-deductible to the extent permitted by law. 

5.  Donations that are unbid, unsold or unclaimed in the Made With Care auction will be retained by The Mother Ship for future fundraisers or mission activities.
