By submitting this form I agree to the following: PHOTOGRAPHY: I hereby grant Off We Go Preschool permission to take and use my child's photograph in connection with summer camps/nature hikes, and for use in school displays, portfolios, publications, and website posts related to the preschool, without payment or any other consideration in perpetuity. These photographs are used for internal communication and projects, promoting the preschool, and as shared content amongst preschool families. Liability Release Agreement: I verify that my child is covered under my own or my family’s medical insurance policy. I do not and will not hold Off We Go Preschool, its owner or employees, responsible for any injury occurring before, during, or after summer camps/nature hikes. Furthermore, I give permission for Jennifer Pulley or other employees to administer first aid and/or CPR to my child, and to call an ambulance and/or paramedic in case of emergency. The undersigned recognizes and acknowledges that there are certain inherent hazards that come with attendance at summer camps/nature hikes. The undersigned hereby (1) knowingly and voluntarily assumes full responsibility for such hazards and risks, and (2) agrees to be bound by all terms and conditions of Off We Go Preschool. The undersigned agrees as a condition of participation in programs of Off We Go Preschool to release, defend, indemnify and hold Off We Go Preschool, the home owners and employees harmless from and against any and all loss, judgments, damage and expense incurred by reason of any claim or liability based upon personal injury or property damage.
