Transcript Request
This form can be used to request Initial Transcripts and Final Transcripts.  We will send these request after final grades are in, which is around the 2nd week of June.

After you fill out this form you will need to go to SchoolPay and pay $2 for each transcript requested.  (Number of transcripts x $2). https://www.schoolpay.com/pay/for/Transcripts/SiZTWL   If you qualify for free or reduced meals you do not need to pay.  If you have any questions or concerns about your transcript request you can contact our counseling secretary, Leann Hollenbeak, at hollenbeak_l@4j.lane.edu.
