Fill out this CoS-SSC Advising Google Form if:

You have recently been disqualified from the College of Science: A former College of Science student recently disqualified from their major and now having assigned an “Undeclared” academic status.
You have recently been disqualified from San Jose State University
You need an Open University Hold on your account removed
You are a former student returning to SJSU

CoS-SSC defines a student as a “Former SJSU Student (FSR)” when they meet one or more of the following criteria:

Former College of Science students who have not attended SJSU for more than one semester and are seeking readmission to SJSU as a science major.
Former College of Science students who were disqualified from the SJSU and now seeking reinstatement and readmission to SJSU or other CSU campuses.

A CoS-SSC Advisor will reply to your Advising Google Form responses between 5-15 working business days after your submission date. During peak advising season, it may take us closer to 15 working business days to reply.

If you would like a Zoom appointment, you will be able to indicate your preferred availability on this form.

CoS-SSC will be unable to respond if you do not type in your email address accurately.
