PUBLICITY CONSENT AND RELEASE

             I grant Rise Together permission to create, store, edit, use, and reproduce Zoom recordings containing my child's likeness. Rise Together may use content for internal operations, training, and pictures on social media to promote programming when appropriate.

             I release and agree to hold Rise Together and its partners, employees, agents,
successors and assigns, from and against any and all claims, liabilities, demands, actions, cause of
actions, costs and expenses, whatsoever, at law or in equity, known or unknown, anticipated or
unanticipated, suspect or unsuspected, which my child ever had, now have, or may have in the future,
by any reason, arising out of or relating to the decision to use or not use my child's likeness in
connection with Rise Together.

             I acknowledge that I have read and understand this Consent and Release
prior to signing it. I certify I am the parent or guardian of (fill in student name below) and have the full power, right, and authority to enter into this consent and release.
