If you are interested in playing Freshman Basketball we will be having Summer Evaluations soon!  They will be held on June 16th, 17th, and 18th from 10:00-11:30 am at Hoover High School in the Competition Gym.  Please fill in your information on this document.

IMPORTANT INFORMATION

Prior to evaluations, all 2020-2021 DragonFly(Hoover High School Code UQ86FJ) forms need to be completed and current insurance card uploaded.  If physical expired in March, April, May, June, or July it will be honored through July 31st.  

Due to COVID-19 Guidelines, you are required to:

You must ever the competition gym through the back door which is across from the soccer field.  A coach/athletic trainer will be there to check your temperature prior to entering gym.
Bring your own water/sports drink
Must have a secured ride ready to pick you up at 11:30 am each day.  No hanging out after practice sessions.
Bring a mask to wear in the building.
