Design Questionnaire for Custom Website (Squarespace)
Please answer the rating scale questions based on how much like one design vs. the other you would like your site to be and provide other information to help me draw up a basic design to prepare for you for the 45 min. consultation to approve/disapprove choices.

***Before purchasing service, please ensure that we are able to meet roughly 2-4 weeks in advance from the date of submission. My booking link is here. Please message me if you have any issues as we can try to find a different time. At our 25-30 min meeting, I will plan to have your site 80-90% complete so that you have a very good idea of what it will look/function like and can approve/disapprove for site to be complete in the next week. Keeping up with the Google Doc is crucial. I recommend checking it every 24-48 hours for updates (with exception of weekends) so we can stay on track.***

I have read, understand, and agree to the above statements.  *
As mentioned in my disclaimer, I am not trained in website laws/regulations, so if you want to ensure your site is up to compliance standards, consult an attorney. I am just offering a design service.

Additionally, there will need to be a few add-ons such as terms & conditions, privacy policies, etc that are on average $14-28. I purchase these upfront and will invoice you for them.

What 2-3 descriptors are most important to you when it comes to your website design? You can also add your own words by clicking "Other." *
Required
If you already have a URL purchased, please enter it here. If not, please type your ideal URL.

(specify if it is NOT purchased yet, please.)
*
Do you have any colors in mind?

If so, please provide Hex codes (or sometimes called HSL or RGB codes) if you can or email me a picture and I can try to retrieve those from the image. Squarespace allows a max of 5, but I would recommend reserving one or two of those to be black and/or white. The colors will need a good amount of contrast, so keep that in mind, especially if you don't want to use black and/or white.

I can also give input, based on your responses.
Do you want your website to be all on one page or multiple pages?

For example, my homepage is one page made up of different sections basically catering to new clients, and separate pages for clients (portal, crisis info, etc.), and separate pages for my professional services (consulting, website, etc.)

We can always discuss this more if you are unsure right now.

Please add contact info for your practice such as phone number, fax, email, address, etc. Whatever you want to be on the website for contact purposes.
What is your ideal demographic? (i.e. kids, teens, people who are spiritual, adult men, etc.)
Do you have any social media accounts that you want to link to this page or a blog? Please provide social media links, if so.

The following questions are meant to aid me in writing an "about me" section and various other text throughout the site.

Resources to aid with some of the questions:

What does your ideal client want from therapy? Be as specific as possible & of course make sure its something you feel able to give to them. (i.e. solutions regarding procrastination, a supportive presence as they navigate life adjusting to a recent loss, an objective perspective as they figure out whether or not they want to stay in their marriage). *

How do you conceptualize the change process in humans? What barriers is your ideal client facing to change by themselves? What can you do to help them with that (can be specific therapeutic modalities)?
What do you hope your clients gain from therapy with you?
