Test Score Release Permission Form
In order for college entrance exam scores (SAT and/or ACT) to be mailed with high school transcripts to prospective colleges, students must give permission for the scores to be included.

At the time of the transcript request, ALL scores on file will be sent.

Even though the scores will be sent with the official transcript, some colleges may request that you authorize the college testing companies (SAT and ACT) to send official score reports to the college's admissions office.

If you give Caledonia High School permission to include your scores so that college application procedures will not be unduly delayed, sign the statement below.
