How old are you? You can apply for this program if you are between the ages of 16-30. Most of the spots are reserved for people who are 16-25. *

Tell us a little bit about yourself and your arts practice. Have you ever taken any arts classes or participated in an art project (Even if it isn't dance or performance related)? Are you a a dance or theatre student? What is your level of experience with dance or other arts practices? For example, have you ever danced before? Have you taken some classes? Do you perform professionally? All levels of experience are welcome! *
Why do you want to be a part of Project Disruption? *
Project Disruption is creating a closed space for cross-disability collaboration and creation. The space is designed for new, emerging, and experienced artists who identify as Deaf, disabled, Mad, neurodiverse, blind/Low Vision, and chronically ill. Do you identify with one or more of these communities? *
Please tell us as much about your disability as you are comfortable talking about. You do not have to include your diagnosis, nor is a formal diagnosis required to be a participant. We just want to make sure we have a mix of disabilities in the group, so some general information is enough. For example, you can tell us a broad category of disability like chronic illness, or Deaf. Let us know if you have more than one kind of disability. If you are selected for the program we will ask you more questions to make sure we can meet your access needs.
Let us know if you have any other identities that are important to you? For example, if you are queer, Indigenous, Black, or a Person of Colour, have experience of homelessness, or being in care etc. We only have a limited number of spots and want to make sure that those who haven't always had access to these opportunities get priority.
The project runs every Wednesday from 6 pm -9 pm, from June 12, 2019 - September 18, 2019. There are three weeks off on July 17, August 14, and September 4th. Please check off below the weeks you will be able to attend. You don't have to attend each week but priority will be given to people who can come more often. *
