I am aware that there is a cost of $20 per person (Support persons do not pay for attendance)  for the 8 week session. This covers the cost of all materials needed. You will be notified of confirmation of your registration via email, at which time payment is required. *

We (I) are the parent(s) or legal guardian(s) of this participant OR I the participant if over the age of 18, hereby grant our (my) permission for him (her, myself) to participate fully in said program, and hereby release and forever discharge The Special Friends Network, Elaine Cray and any of their associates or volunteers, And the Milton Public Library from any and all responsibilities and liability claims.  We are aware that this participant and/or parents or guardians are fully responsible for this participants well being and safety. If the participant needs one to one care, a support person will attend with the participant.
