San Jose Chinese School (SJCS) is a non-profit organization and is operated under the contributed efforts of all parents/guardians.

- All parents/guardians are required to support, participate and perform voluntary services. Each family is required to fulfill 10 service points per school year. Parents may choose to pay a $150 fee in lieu of service. Failure to comply will lead to penalty such as dismissal of the student.

- SJCS does not discriminate students on the basis of race, color, ethnic national origin, sex or handicap in any of its polices, procedures, or practices; nor does SJCS discriminate on the basis of sexual orientation. This non-discrimination policy covers admission and access to all school programs and activities, including but not limited to academic admission, financial aid and educational services.

- SJCS is not responsible for any medical expenses or accident/injury involving student on/off campus, and to/from school.

- Students will provide their own transportation to and from the SJCS.

- Students must follow all school/class rules and regulations. Student who violates these rules or regulations will face severe consequences. This may result in voluntary dismissal from the San Jose Chinese School.

- Parents should treat all SJCS members with respect. Inappropriate behavior may lead to dismissal of the student.

- Regular school hours are 8:30 A.M. to 12:30 P.M. on Saturday. For safety reasons, no students shall stay on campus 15 minutes before or after school hours unless accompanied by parent or guardian.

- It is solely the parent/guardian responsibility to update any of the information provided by this form. SJCS is expected to be informed of any updates made within two weeks after the changes take place.

- SJCS is working to apply modern technology to its curriculum and daily administration. The school may
photograph/videotape/record our students for use in teaching, school publication, website or other purposes.
I, parents/guardian of the student, acknowledge and agree to the above obligations. I also understand that agreement to these obligations is prerequisite to the admission of our child to the San Jose Chinese School. I, on behalf of my child, myself and my family, hereby waive all our rights, claims and actions which we now have, or hereafter may have, against the San Jose Chinese School and its board members, staff and all volunteers, arising out of, but are not limited to, the above mentioned and other cited class/school activities.

I hereby grant permission to SJCS authorized personnel to administer basic first aid to the student as appropriate. I also grant permission to SJCS authorized personnel to arrange transportation for the student in case of accident or acute illness and to arrange for medical, dental and/or surgical care at the closest hospital in case of emergency. I also understand that an effort will be made to notify us before such action is taken and all expenses will be assumed by us.

I hereby grant permission to SJCS to take photographs, video footages, and audio recordings of my child in SJCS related activities and to use them in its websites or other media published by SJCS.

I have read and fully understand the meaning and consequences of the foregoing waiver and thus voluntarily execute this waiver accordingly.
