Please can you (the guardian) fill out sections one to three and five, allowing players to use their own language to fill out section four. Closing date for the application (including video) is 31st July 2023. Failure to send in both will mean we will not be trialing you for 2023/24.

All players must fill out the form and send their videos (even if you are in a current SYC squad).
