**Please note - Everyone entering the area MUST complete the screening tool of the arena you are attending for each individual and take a screen shot ready to share at the door only 60 mins prior to entering the facility

Campus - All participants will have to download the Ontario Tech mobile app, fill out the COVID 19 health screening questions and show proof by screen shot on their cell phone or email verification before entering the building. Hard copies will be available at the front entrance to fill out if access to the app is not possible.
