Hello! This form is meant to be filled out if there is an individual within CUDS with whom you are not comfortable debating with, co-judging with, or being judged by. It is most commonly used in situations where participating in a round with an individual would pose emotional harm or disturbance to yourself, and/or you feel that the individual as a judge would not be able to judge you in an unbiased manner due to a pre-existing personal relationship, be it positive or negative.

All requests made on this form are confidential and will only be shared with Emma Callbeck, who is doing the CUDS slotting. If you are uncomfortable with me knowing, please feel free to message another member of the executive, who will uphold the scratch.
