Feb/March 2021 Enrollment Commitment Form
Please complete this Enrollment Commitment Form to select a learning option for February 8, 2021 - March 26, 2021.

Select the Full Remote Learning option if you do not want your child/children to attend school in-person.

Select the In-Person Learning option if you want your child/children to enter the school building for in-person instruction.

Students in our half-day programs for EC, Pre-K, and Kindergarten will attend 4 days, (Mon, Tues, Thurs, Fri) per week either in the morning from 8:30-11:00, or in the afternoon from 12:30-3:00.

Students in grades 1-5 will attend 5 days per week from 8:30-12:30, with an additional 1 hour of remote instructional time in the afternoon.

Students in grades 6-8 will attend 5 days per week from 8:20-12:20, with an additional 1 hour of remote instructional time in the afternoon.

Please complete a separate Enrollment Commitment Form for each child enrolled in any Worth School.

If you select Full Remote Learning for your child, this choice will be in place until March 26, 2021.

THIS FORM IS DUE BY THE END OF THE DAY ON THURSDAY, JANUARY 21, 2021.
