This form is to be completed for each student at the beginning of the school year, and is required for participation in the Union High School Choir program.
IMPORTANT NOTE: Students are to communicate with their parent/guardian that contact info is being collected. It will be used strictly for communications from the Choir Boosters (refer to
www.unionchoir.org/privacy-policy for details of our privacy policy). If there are concerns providing this info electronically, a paper form can be requested.
The first section involves collection of STUDENT information, and subsequent sections are for PARENT/GUARDIAN information.
