PLEASE READ CAREFULLY

To ensure that in-person lessons at Rachel Loerts Music are done in the safest way possible, students wishing to have lessons in-person must agree to the following:

1. The student(s) and parent(s)/guardian(s) must not have any symptoms of COVID-19 including, but not limited to fever, dry cough, tiredness, and shortness of breath the day of the lesson.

2. The student(s) and parent(s)/guardian(s) must not have any cold or flu symptoms the day of the lesson. If the student has a cold they can have their lesson online for that day.

3. The student(s) and parent(s)/guardian(s) must not have had any close contact in the last 14 days with anyone who has tested positive for or is suspected to have COVID-19.

4. The student(s) and parent(s)/guardian(s) must provide their own mask and wear it at all times upon entry of Rachel Loerts Music.

5. The student(s) and parent(s)/guardian(s) must use hand sanitizer upon entry of Rachel Loerts Music.

6. Rachel Loerts Music and it’s teachers are not liable if a student(s) contracts COVID-19 from participating in in-person music lessons.

7. Rachel Loerts Music and its teachers are not liable if a student is injured, becomes ill, or contracts COVID-19 due to the student and/or parent(s)/guardian(s) negligence.

8. By signing this liability waiver you are giving up your legal to right to any future claims of injury or illness due to participation in in-person music lessons.

9. If the student(s) or parent(s)/guardian(s) test positive for COVID-19 and have been at Rachel Loerts Music in the past 14 days, they agree to tell Rachel Loerts who will anonymously disclose the results of the test to other students taking in-person lessons.
