Personal Communication Devices & Internet Use Agreement - Parent
As a parent/guardian, I/we have read and do understand the school district’s policy relating to the use of Personal Communication Devices & Internet Usage in School in the Parent/Student Handbook (Sections 7 & 8 in the ISD511 Parent/Student Handbook).  By checking below, I understand that if my child/children possess a Personal Communication Device (cell phone, laptop, Chromebook, etc.) at school, they are subject to the limitations of this and other district policies.  I further understand that any unauthorized use of these devices will result in confiscation of such device and other disciplinary measures deemed appropriate.

As a parent/guardian, I understand Internet Usage access is designed for educational purposes.  The school district has taken precautions to eliminate controversial materials and I will not hold the school district or its employees or agents responsible for materials acquired on the Internet.  Further, I accept full responsibility for supervision, if and when, my child’s use is not in a school setting.  I hereby give permission to issue an account for my child/children and certify that the information entered below on this form is correct.
