Dear Parent / Caregiver
The Safe Routes to School Program (SRTS) would like to learn your thoughts about how your child(ren) get to school. This survey will take about 5 minutes to complete. We ask that each family complete one survey per school your children attends. If more than one child attends the same school, please fill out the survey for the oldest child. If you have multiple children at different schools, please complete one survey for each child, but still only one per school. (example, if you have 2 children at an elementary school and one at CCMS, please complete one for the oldest elementary student and one for the student at CCMS).

Each week, we will draw a winner for a goody bag from the responses. If you are interested in entering the weekly drawing, please include your e-mail at the end. Your responses will be kept confidential and neither your name nor your child's name will be associated with any results.
