Please fill in the information below to electronically inform the school of your child's absence.  This form will not be valid if completed by a student.  THIS FORM IS NOT USED FOR TARDIES OR EARLY DISMISSALS.  Tardies will only be excused with a physical note from a parent / medical provider or a phone call from a parent.  Early dismissals require a physical note to be brought to the main office by the student on the day of the request.  If an early dismissal is requested by phone or email, the parent must come into the school to sign the student out.  Tardies and early dismissal entries on the parent portal will not be processed.  If you have any questions, please contact the school - 540-658-6115
