This form serves as a legally binding agreement and waiver that is required to participate in any and all Nastaaja Johnson, AKA Coach Coco, and Circus Paint Classes, Events, Rehearsals, Functions, or Activities, hereby and throughout referred primarily as Circus Paint LLC and Circus Paint led Activities.

This form allows Circus Paint LLC to know that any participation is voluntary by the Participant and that the Participant knows that they are assuming all risks that may be associated with participation, including but not limited to contraction of COVID-19.

This form allows Circus Paint LLC to use your captured or recorded image and likeness taken by
Circus Paint LLC, any third party, or by yourself and given to Circus Paint LLC to then be used by Circus Paint LLC to Further promote Circus Paint LLC or feature the Participant.

Typing your initials (section 1-6) and name (section 7) throughout this form will act as signature that you have read and understood all content and that you are in full agreement.

If you are an adult completing this form in on behalf of a young person under the age of 18, then you are assuming all risks, accountability, and liability laid out in this form for that minor.
