Thank you for registering for Marlene Meyerson JCC Manhattan's upcoming Food for Life program, The Power of Plant-Based Nutrition for Health, presented by Plant Powered Metro New York (PPMNY) and conducted on Wednesdays, July 14 and 21 & August 4 and 11, 2021.

Before we get started, PPMNY would like to collect more information from you to better understand your perspectives and experiences. We'll ask you to complete a similar questionnaire at the end of the program. This data will be used to help us understand the demographics of our group, each person's starting place, and, later, the impact of your experience. All responses will be reported on in the aggregate to share about the Food for Life program's impact with our community, in future marketing materials, or to funders.
