Thank you for applying to be a chaperone at the 2020 Vision Protestant Reformed Young Peoples Convention. We are thankful for your desire to give of your time to make this a God glorifying and enjoyable time for our young people!
Before continuing, if haven't done so, go to
prcconvention.com/chaperones and please read carefully the Chaperone Guidelines and Objectives and Methods of Chaperoning a PRYP Convention. You will be asked at the end of the online application if you have read them and agree to them.
Just a few things to remember when applying to be considered:
1) Online Application
Complete the following online application form, it is three pages after hitting 'Next' below.
Once complete, a confirmation email will be sent to the email address you provided.
2) Medical Form
After online registering, go to
prcconvention.com/chaperones and download the Medical Form.
Print the Medical Form and complete all required information.
..... or 'Right' click the website link, Save As, fill out and electronically sign and date
3) Insurance Card
Submit a picture of both the front and the back of your insurance card
4) Approval Letter
Request a letter from the clerk of your consistory stating your good standing as a member of your church
When you are ready, email a picture or PDF of each document from steps 2, 3 and 4 to:
