Seinfeld/Wennberg group culture feedback
This is a way for students/postdocs/staff working with John or Paul or other professors to provide anonymous feedback about the group culture and ways to make everyone feel included, valued, and safe. We will address all issues brought up in this form, but our role is primarily to bring up issues to John and Paul and to together brainstorm ways to improve group culture. If you would like to bring up a Title IX Violation, there is contact information here (including confidential resources): http://www.titleix.caltech.edu/FAQ

The designated ombudspeople (Josh Laughner and Sophia Charan as of June 23, 2020) will bring these concerns to John and/or Paul and/or other professors mentioned.

This form will at first be seen solely by Josh. If you don't want information shared with Sophia, please answer accordingly. If you would to submit this form to Sophia and not have it seen by Josh, please fill out this form instead:

What is your suggestion? Is there some general change you feel would make our groups more welcoming, or a specific incident that should be addressed? *
Which PI(s) should this be brought to? If you feel that answering this question removes too much anonymity, then don't feel you have to answer. In that case, the ombudsperson will automatically bring it to both PIs.
If you answered "Other" to the last question, are you okay with the ombudsperson enlisting Paul or John for support in contacting that PI?

Josh and Sophia are both acting as ombudspeople and will jointly bring these issues to Paul and/or John. If you prefer that only Josh sees/brings up these issue, please indicate that here.

All responses to this form are anonymous and Josh will get an email as soon as a response is submitted. Josh and Sophia will bring concerns up with Paul and John about once a month (so that concerns are aggregated), unless there is something that is time sensitive. If you would prefer for Josh to contact you and let you know the feedback/resolution from John/Paul, you can put an email address here (or an anonymous email address) and he will contact you. Otherwise, please just leave it blank.
