Please use this form to request an invoice for any ONE of the offerings from the USAWE Licensed Officials Committee.

Once submitted and approved, this form will generate an invoice through the USAWE PayPal account, which will be emailed to you.

For some of the requests (such as an application to become a Judge or TD, or to apply for a promotion), you will also need to submit a complete application to the LOC (LOC@usawe.org), but in all cases, filling out this form to request that an invoice be sent to you is the first step. DETAILS ARE FOUND IN THE LICENSED OFFICIALS PROGRAM
