Junior High Youth Ministry Registration 2021-2022
Please fill out the following form to register your teenager/s for junior high youth ministry for the 2021-2022 school year! Junior High Youth Ministry meets on the 1st and 3rd Wednesdays of the month from 3:30-4:30pm in the new Parish Life Center! We will offer additional events throughout the year -- so be sure to stay tuned in!

As youth ministry is an after school activity that embraces the responsibility of maintaining a safe gathering space for all, we will be following the school's mask policy that mask wearing is required for all small group activity inside, but masks can be removed when we are outside and are able to maintain distance. As is the case, we will try to meet outside as much as possible when the weather permits.

Please remind your teenager of good hygiene practices and it's always great to have a personal bottle of sanitizer on hand! Also, if your teenager is not feeling well or if anyone in the family is showing Covid-19 symptoms, please stay home and focus on feeling better! With Zoom technology, meeting from home is always an option we can setup for you!

I/we waive all claims of any kind against any or all of the organizations or persons hereinabove enumerated, including any all claims against persons transporting aforesaid participant to or from activities hereinabove named. *
Do you hereby give consent to photograph or videotape aforesaid participant and without limitation to use such photographs or videotapes and or stories in connection with youth ministry for the sole purpose of any work of Saint Albert the Great, the Youth and Young Adult Ministry and CYO Office, and I do hereby release Saint Albert the Great, the Youth and Young Adult Ministry and CYO Office from any claims whatsoever which may arise in said regard. *
