Ashbourne Ladies Hockey Club  Membership Form 2023-2024
The data being collected will be kept in a secure location and will only be accessible to select club members. Please only complete this form if requested to do so by a member of the club committee. Any information given will be solely used by Ashbourne Hockey Club and will not be passed on to any other third parties - Visit the club website to read our Privacy Policy.

Fees this year have increased but they now include the cost of training meaning a saving overall. The new fees are as follows:

  • Adult - £90 per season

  • Full-time students/juniors (up to 21) - £50 per season

  • Family/Household - £110 per season

  • Social membership - £20 per season


Note: Family/Household membership is 2 or more members from the same family/household (1 adult maximum, no limit on juniors e.g. 1 adult with 2 juniors/students, or multiple juniors/students with no adult).

Annual Subs
There are a few options for paying annual subs:

• Direct Debit 
• Standing order – 8 monthly payments over the season, starting September 2023 ending April 2024. This must be agreed in advance with your captain.

We kindly request that you use on of the above methods if possible

• Cash or cheques (payable to Ashbourne HC - Ladies), which can be given to your captain.

Membership fees are due by November 1st 2023 unless agreed otherwise by your captain.

Please note players are not insured until a payment has been received.

If the member is under 18 the following must be completed by a parent/guardian
