Parents/ Guardians:

For students ALREADY ENROLLED at KVS: Please complete this form for your elementary student for second semester.

For students NEW to KVS: Once you have received your acceptance letter from Kelloggsville Virtual School you may complete this form to secure your classes for the Second Semester.

STUDENT NAME (Last, First) *
Check the box the of the grade level the student will be entering for the 2022-2023 school year.  This will automatically assign the proper English, Math, Science,  Social Studies, and elective classes. *
Every student in grades K-5 must be assigned an "At Home Learning Coach". Please state the person's name who will be helping your child. (This person must be over the age of 18 and not enrolled in a K-12 school). *
What is the relationship of the At Home Learning Coach to your student? *
IN PERSON LEARNING SESSIONS: Please choose as many sessions as you would like your child to attend at KECLC.  Bussing will be provided for those that qualify within Kelloggsville School District. These sessions will be held daily from 12:15pm-2:00pm. Monday-Friday. *

The State of Michigan requires all elementary virtual students in grades K-6 have an Education Development Plan on file. This is a  series of 5 questions that will give us information on your child's interests in order to provide more understanding on careers for them.  Ms. Taylor can hold these meetings virtually or over the phone with you. Please check a day of the week and time frame that works best for you.

Elementary Intramural Options (Grades 3-5): Please Check any sports opportunities that your child is interested in participating in. Mrs. Taylor will send this information to the Central Kelloggsville Athletic Department for them to contact you regarding that sport. All athletes will be required to have a current physical on file before their first official practice. *
