If you have a concern about a class or a teacher, we urge you to contact that teacher directly to attempt to resolve your concern. If, after meeting with the teacher, you still have questions, concerns, or feel that more support is necessary, we ask that this form be filled out so that the Leadership Council may work with you and the teacher in order to come to resolution on the matter.

Once you have completed the form, we will respond to your request within the span of two business days. Thank you for your time and patience as we work to resolve your concern.
