CHOICE STUDENTS:  If you are accepted to IB and join the program, if you exit, you will have to return to your zoned school.

If you are Out of Zone, you must complete a Parent School of Choice Form (a Form 407) and a Signature Request form (407C).  When the forms are complete, please send them to the IBC at SFHS via email, snail mail, or as a scanned attachment.  PLEASE do not send these forms to the District office.  PLEASE send them to the IBC at SFHS @ riccark@martin.k12.fl.us.   These forms are available from the district, or any guidance office, or you can click this link:https://drive.google.com/open?id=1xS95SLGbV-XtUT8pVEsetuOYphx860EO.  By submitting and signing this form, the parent and student acknowledge that should the student be accepted and join IB, then decide to exit, the student will return to the zoned school.

In order to meet the demands of this program, it is necessary for a student and parent or guardian to understand and agree to the terms of the Admission Agreement. Statement of Intent (to be signed by parent/guardian): I (we) have read  the International Baccalaureate literature and commit full support to my/our/son's/daughter's participation in the program, including academic endeavors and Creativity, Action and Service hours. I will ensure that my child has uninterrupted  study time daily and will supervise the use of study time in the most productive way. I will also commit full support to the IB faculty and administration by following their guidance regarding further study in order to ensure my child's successful completion of the IB diploma program at SFHS.
