I understand this application is for the 23/24 school year, which begins August 10, 2023. *
*
I understand that CVS will review all applications to ensure that a virtual learning environment is appropriate before accepting students. Virtual education is not appropriate for all students.
*
I understand that CVS is a Program of Choice. Parents of full-time CVS students are responsible for transporting their students to and from our school for state/district assessments and progress monitoring. We will be located at the Charlotte Town Center Mall.
*
I understand that CVS students do not meet with teachers each day. All curriculum content is provided online.

*Elementary students have supplemental Zoom sessions weekly. Parents serve as a full-time learning coach and elementary students require a parent to assist them in working through the program on a full-time basis. Online learning with CVS is a family commitment.

*Secondary students do not interact with their teachers on a daily basis. Students must be highly motivated and responsible. Students must have the ability to complete their work independently in order to be successful in the program.

Has your child previously attended Florida Virtual School (FLVS) or Charlotte Virtual School (CVS)? *
CVS students in grades K-11 are required to maintain a minimum of six courses per semester. Seniors are required to take graduation requirements, which may be less than six courses. Students must maintain adequate weekly progress as defined by a minimum of 5% completion in each course every week. Students who do not continuously submit work and stay on pace, run the risk of being withdrawn at the end of the semester.  *
Is your child identified as an ESE or 504 Student?  *
I understand that if my student currently receives services through Exceptional Student Education (ESE), placement in a virtual program requires a review of the student's Individual Education Plan (IEP) with the student's assigned school location prior to enrollment into Charlotte Virtual School.

*Families are responsible for scheduling the appointment with the assigned school and inviting a CVS representative to attend.*
