I will be attending the Forestbrook Students' 2022 Fall Retreat starting at 6pm on Friday November 4th through 12pm Sunday November 6th.  *
I attend Forestbrook Student Ministries regularly  *
If you answered no to the previous question - please indicate who you are attending with. So we can make sure that you will be roomed and grouped with someone you know. 
I understand that once I arrive I will not be permitted to leave until the event is over. Any scheduling changes need to be discussed and approved by Pastor Nathan and my parents before the event begins.  *

I understand that any and all medications, supplements, vitamins, etc will be secured and administered by Forestbrook Baptist Church, no medications will be allowed to stay with students over the weekend. 
We will have a "Friends Giving" meal Saturday Night where parents are not only invited, but encouraged to come. We would love for you parents to come for dinner and even stay for Saturday Nights' worship and message.  *
What should I bring? 

1.Appropriate Clothing (including clothes for church Sunday Morning and clothing to sleep in - no tank tops) 
2.Bible, Notebook, and pen
3. Toiletries 
4. A pair of cloths that can get messy / wet 
5. A Pair of close toed shoes


What I should not bring :

1. Tobacco or vaping products of any kind 
2. medications or vitamins that are not turned upon arrival 
3. Weapons of any kind

I understand that the goal of Forestbrook Student Ministries is to create a safe and encouraging environment for everyone involved. My behavior should reflect these same goals. Any harassment or bullying (both physical or verbal) will not be allowed and will result in Pastor Nathan contacting my parents - regardless of time of day- and my parents will be required to come pick me up and will effect my ability to attend future events.
