Your counselor will use the information you provide in this questionnaire to compose your letter of recommendation. You will need 30-45 minutes to thoroughly complete this questionnaire. Please take your time and share as many details as possible. More information is better than less, and minimal information will result in a weaker letter of recommendation. Anything you share in this questionnaire may be used in your letter of recommendation. After submitting this questionnaire, please set up a meeting with your counselor. Counselors will not write a letter of recommendation without a completed student questionnaire and a student/counselor meeting.
