The purpose of this form is for you to communicate concerns of a serious, critical nature, so that we may better understand your child.  It is for parents/guardians who:

1. Want to share a significant emotional, social or academic issue, which needs to be considered before placement.
2. Want to share with the Department Head of School Counseling  a sensitive situation that may affect their child’s transition to the middle school.

Because our placement process goal is to form well-balanced homerooms, we cannot consider requests for specific teachers, teaching/personality styles and/or requests to be with friends.  Note: this form will only be used for the 2024-2025 school year.
