NAH Student Clinic Registration Form

Thank you for your interest in our NAH Student Clinic - Let's get you registered for one of our next available openings. Tell us when you are free and we will be in touch when an appointment becomes available!

First: Read the list of available intake appointments below. Check ALL OF THE TIMES that could work for you. Appointments are typically available on Fridays and Saturdays. We will be in touch via email when a date of your choosing matches one of our openings. At that time, you will be booked! Until then, you are registered and awaiting confirmation of an available appointment time. Please be patient, as getting confirmed for an open time can take several weeks as our low-cost clinic is busy.

Second: Complete all of the necessary information in this form, digitally accept the NAH-MNR Student Clinic Payment Agreement below, and press "Submit"! Appointments are filled first-come, first-served. Once a time is assigned, we will invoice you via your email before or just after your appointment. Please watch your email for this invoice and pay upon receipt to secure and ensure future bookings with NAH Student Clinic.

What  happens after I press "Submit" below? An NAH staff or student case manager will be in touch with appointment instructions, paperwork, and answers to any questions you have about attending NAH Student Clinic. As a newly scheduled client, it is imperative that you respond to the case manager's communications, requests for forms, and all NAH/MNR invoices in a timely manner. Prospective clients who do not respond will be removed from the schedule and will need to submit this interest form. 

After submitting this form, if you still have questions about the NAH Student Clinic experience, scheduling or billing you will find our our contact information in the form's confirmation email. Keep this email - it also has a copy your responses so that you can update your responses as your availability changes. Thank you! 


NAH Student Clinic Appointment FAQ's
  • Appointments are available via Zoom video call only unless otherwise noted (see Available Appts below)
  • Initial intake appointments range between ​1.5 - 2.5 hours. Homeopathic intakes are very in-depth, consisting of a holistic exploration of any physical, mental, or emotional symptoms. This holistic approach enables us to select homeopathic remedies best able to support you and your wellbeing. Follow-up appointments are typically 1 hour in length.
  • Most appointments below are listed as "NAH Clinic..." These are live in front of a small clinic group and are typically completed online (unless noted otherwise). They are staffed by advanced students and supervised by trained faculty. All other appointments listed as "Classroom..." are live in front of an in-person class with your case being taken by trained faculty. These appointments are in-person in our Edina, MN  classroom (unless noted otherwise).
  • FeesInitial consultation appointments: $89Follow-up appointments: $49Between-appointment calls: $29Payment is due to NAH by time of booking, payable via NAH's secure online invoicing system (see above for details0. Please watch your email for your invoice and pay upon receipt.
  • Remedies: All NAH-recommended remedies will be ordered for you through Minnesota Remedies, Inc. (MNR). MNR will invoice you directly for these via their secure online invoicing system. Fees range from $8 - $35 plus shipping for remedies. This invoice is separate from your clinic appointment invoice.

NAH & MN Remedies Student Clinic Payment Agreement

To ensure affordable and responsive care, Northwestern Academy of Homeopathy (NAH) and its subsidiary, MN Remedies, Inc. (MNR), require that NAH student clinic clients pay NAH in full for their NAH student clinic appointments and pay MNR in full for any recommended remedies at the current rates.

Clinic appointment fees are due to NAH upon invoicing and are subject to our 72-hour cancellation fee policy (non-refundable when canceled less than 72 hours before appointment start time). Recommended NAH Student Clinic remedies will be ordered by your NAH Student Clinic supervising faculty through MN Remedies, Inc. (MNR). You will be invoiced by MNR for these remedies. Paying each of these separate invoices will allow your remedy to be issued to you. NAH and MNR use separate, secure online invoicing systems for you to conveniently pay for fees and remedies.

Filling out this form will give both NAH and MNR the necessary information to bill you separately for their services/products. If you have booked a clinic visit or are expecting a remedy, please look in your email for invoices from NAH and MN Remedies. Paying each invoice promptly via the secure links in the invoice will allow us to get remedies to you and continue your care with NAH Student Clinic. 

Once both invoices are paid, MNR will send the recommended remedy to the name and address entered below. Information on this form is kept securely and confidentially at each organization. Should the information you enter below change in the future, you are responsible for updating your NAH case manager of any changes.

In your online invoices you may have the option to choose to securely store your credit-debit card for future charges. Doing so assumes your authorization. You may remove this card from file at any time.
