Please use this form to sign up for a 2022-23 photo book.  I ask for advance signup in order to collect photos of your player throughout the season which saves enormous amounts of time at the end.  Payment of $30 ($40 for late signup) for curating and creating the book on mixbook.com will be due at the end of the season.  If your player does not get significant playing time, I'll let you know whether I think there are adequate photos to make a nice book before you pay.  (Once it's created, you'll purchase directly from mixbook - most football books last season were $40-80.)
