This form is to be used by the parents/guardians AND students of the 2019-20 Streetsboro High School Marching Rockets.

Before submitting a request please note that ALL rehearsals and performances (Band Camp, Wednesday after school rehearsals, football games, band shows, parades, etc.) are required/mandatory attendance for ALL members of the Marching Rockets.

This form is to be used in the RARE case a student is unable to attend a rehearsal and/or performance.
