Youth Spring Ball Hockey -  For ages 5 - 18.  This is by INDIVIDUAL registration. Format will be 4 on 4 (could change depending on numbers).  The surface will be split in half and games will be played on half-ice. Games are held in the Spiritwood Skating Arena on the concrete slab. Registration is $30/child. All participants need running shoes, hockey stick, gloves and helmet. Goalies require full hockey gear that will be provided (pads, chest protector, catcher & blocker, goalie stick)

RELEASE AND INDEMNITY AGREEMENT
I, I am the parent or guardian of the minor, I will ensure that the minor complies with all rules respecting the Program and I assume and accept all risks, dangers and hazards in connection with the minor’s participation in the SARCS Youth Ball Hockey Program.
2. I release Spiritwood Area Recreation, Culture and Sport Inc. from and will indemnify them against all claims, losses, injuries and liabilities whatsoever by reason arising out of the minor’s participation in the program, due to any cause whatsoever. I will not make any claim or commence any legal action against Spiritwood Area Recreation, Culture and Sport Inc. by or on behalf of the minor.
3. This agreement shall operate for the benefit of Spiritwood Area Recreation, Culture and Sport Inc. and their respective successors, heirs, executors, administrators and assigns and shall be binding on me and my heirs, executors, administrators and assigns.
4. The Spiritwood Area Recreation, Culture and Sport Inc. may use my child’s photos in promotional programs including, but not limited to, newspaper articles and press releases, grant follow-ups, Northern Lakes Community Calendar, Town of Spiritwood websites and any official SARCS promotional material.
