Parents/Guardians,

It is important that students are in school daily to ensure they are learning and prepared to successfully complete their current grade level. If your child must be absent, please complete this form. Although absences can be excused, the total number of absences can still affect a child's promotion to the next grade. After 3 consecutive absences without a reason, parents will be required to meet with an Attendance team member. If you have any questions, please contact the main office at 773-535-1757. Please complete this form for each child that was absent.
