2) Bear tag

3) Town of harvest

4) Wildlife Management Unit

**After this registration is completed a notification email will be sent to the responding Conservation Officer. Please note the completion of this form is NOT in lieu of properly registering/sealing a harvested bear with a Conservation Officer. This form is strictly for the purpose of contacting a Conservation Officer to report a bear harvest. A Conservation Officer will contact you within 24 hours (often sooner). It is important that hunters take appropriate steps to preserve/cool their harvest in the meantime.**
