Instow Key Worker Questionnaire - From 8th June
This form is intended for key worker families who have not already informed the school of their need for provision.

Unless you have completed a form previously, and we are already aware of your circumstances, you must complete this form in order for your child to attend Instow Primary and Pre-school from Monday 8th June.

We are offering provision for breakfast club if needed (with a homemade packed breakfast sent in for your child) as well as the core hours of the school day.

Please complete one form for each child.

Who is eligible:

Parents of three and four year olds from pre-school as well as Instow classes of designated key workers or EHCP pupils WITHOUT a pre-existing medical condition.

It is not compulsory for key worker children to attend school, it is offered on a voluntary basis.

The government has asked parents to keep their children at home, wherever possible, and asked schools to remain open only for those children who absolutely need to attend. From Monday 1st June, this has changed to allow Pre-school, Reception, Year 1 and Year 6 children to begin to return to school.
