We invite you to take part in this short survey to tell us about your experiences of engagement with Health Services as an Older member of the LGBT Community.

We have developed this short questionnaire in line with the HSE patient experience survey of interactions with the primary care team to allow us review the experience of older LGBT+ people engaging with healthcare professionals in these settings compared to the general population.

What is a primary care team?

A primary care team is a team of healthcare professionals who work together to meet people’s
health and social care needs locally. A typical primary care team consists of:

• GPs,
• nurses,
• physiotherapists,
• occupational therapists,
• social workers, and
• home-helps.

They all work together to share information and coordinate skills so that you receive the best
possible healthcare available.
