Outdoors at UVA Misconduct Reporting Form

If you have experienced any misconduct by a member of Outdoors Club, or outside of the club with a club member, please fill out this form. This includes harassment, discrimination, sexual misconduct, unsafe driving for club trips, and/or any other behavior that you believe is out of line. If you are not sure whether a specific behavior needs to be met with disciplinary action, report it anyways, especially if you believe it may be categorized as sexual misconduct. Outdoors at UVA has a zero tolerance policy for sexual misconduct, and we want to make sure all club members feel safe attending events and trips.

We ask for a lot of information in this form. Please answer all the questions you are comfortable with & with as much detail you feel comfortable sharing. The more information we have, the more thorough our investigation & response can be. We want to take action as professionally as possible, and work with you to address the situation in ways that you feel comfortable with. The only person who will see the responses to this form is the club's DEI Chair, and the President if necessary. If there is a conflict of interest, decision-making will be deferred to the Vice President and/or previous DEI Chair.

Keep in mind there are multiple courses of action we can take regarding your situation. This may include prohibiting the perpetrator from attending trips/events that you are on, or removing them from the club entirely. Our goal is to take the action that will make you feel the safest, and we will work with you to get to that place.
