Camp is June 15-19, 2020. The cost of the Camp Invention Before & After Care is $95 for the entire session with unlimited hours from 7:30-9:00 am and 3:30-6:00 pm, or $55 for morning care only.

Before & After Care parent acknowledgment form *

I acknowledge a NSF fee of $25 will be charged for all returned checks. Following a returned check, future payments must be made in cash or with money order.

It is the responsibility of the enrolling parent to provide legal documentation if a child is not to be released to a noncustodial parent or other individual.

The cost of the program is $95 for the entire session with unlimited hours from 7:30-9:00 am and 3:30-6:00 pm or $55 for morning care only.

I acknowledge staff will ask for identification each time I or an authorized person picks up my child.

I understand that my child may not bring toys, games, electronics or cell phones to be used/played with in before or after care. Farmington Public Schools is not responsible for belongings that my child brings into the program.

I acknowledge that the before & after care hours of operation are 7:30 AM-9:00 AM and 3:30 PM - 6:00 PM Monday through Friday

I acknowledge that picking my child up late (after 6 PM) is subject to a late fee of $1 per minute per child
