Grove City Area School District Survey for the 2021-2022 School Year
The following form is to be completed by parents/guardians on behalf of their children.  Please complete one form for each student as we work to collect information on parent/guardian preference at this time regarding next school year.  If you have more than one child, you will need to click "Submit" and then begin a new form for each additional child.

This is a preliminary survey for next year, which means parents/guardians can change their preferences over the summer.  We will also be sending out another survey during the summer months for our families to solidify their requests.  The information collected through this form helps the district immensely as we prepare.
