CSMS Athletics Sport Event Student Early Release/Parent Pick Up Form 23-24
Parents/Guardians of Sandburg Panther Student Athletes who wish to have their student released directly from the event into parent/guardian care must complete this form at least 24 hours before the Meet event. Submit one form per meet event per student. Following this procedure will help with team management and the safety of all student athletes. 

Please note:
  • While emails are welcome, this form is the required method of communication to pick up your child from the meet in lieu of county provided bus transportation back to Sandburg. 
  • Students may only be released to their respective “on file” guardian / parents.
  • No “buddy/neighbor” releases permitted.
  • Parent must complete this form and physically sign out their student athlete on the Sandburg Team Clipboard. 
  • Please be prepared to show a government issued ID when signing out.
